Abstract: 

Unilateral temporomandibular Joint (TMJ) dislocation is a rare condition often triggered by yawning, vomiting, or other activities involving wide opening of the mouth, resulting from the forward movement of the condylar head. This case report highlights a longstanding unilateral TMJ dislocation managed by the single injection of local anesthetics into the deep temporal and masseteric nerve peripheries, alleviating muscle spasm and pain to restore TMJ function.

Abstract: 

Introduction: The origin of 16% to 30% of low back pain is sacroiliac Joint. It is difficult to distinguish the sacroiliac Joint pain from other types of low back pain using history and physical examination. Radiological imaging has little role in diagnosis of the sacroiliac Joint pain and diagnostic Blocks are the gold standard in these patients. The main objective of this study is to evaluate the accuracy and safety of diagnostic sacroiliac Joint Block (DSJB) under sonographic guidance.Materials and Methods: This analytic cross sectional study was conducted on 65 patients. DSJB under sonographic guidance was done for all patients with 1 ml radiographic contrast material and 1ml local anesthetic (2% lidocaine). In order to confirm intra-articular injection by sonographic guidance, fluoroscopic spot images were considered. The side effect and positive predictive value of physical provocative tests of sacroiliac Joint were evaluated.Results: Of 65 diagnostic Blocks, after fluoroscopic confirmation, 62 injections were intra-articular and only 3 were peri-articular. There was a significant statistical correlation between procedure time and patients' body mass index. 3 and 4 to 6 positive provocative tests of sacroiliac Joint resulted in a positive predictive value of 83.3% and 93.3%, respectively. The procedure had no side effect and complication.Conclusion: Based on the obtained results, diagnostic Block of sacroiliac Joint under sonographic guidance has reasonable accuracy without side effects. If performed by experienced clinicians in nerve Block under sonographic guidance, it can be valuable alternative method for other guidance modalities in diagnosis of patient with sacroiliac Joint pain.

Abstract: 

Alarm systems are essential in safe operation of industrial plants. Since many process variables are interacting with each other, so in this paper, an approximate method is introduced to design and analysis of a multivariate alarm system. In this method, the alarm system is designed base on Joint indices. The Joint FAR and Joint MAR are defined for a m-variable alarm system thanks to multivariate Markov scheme. In proposed method, the alarm Joint indices are defined by solving a Linear Programing (LP) optimization problem. By defining Joint indices, tuning of the alarm parameters (like, threshold and etc. ) can be done by these indices instead of correlation analysis. In this paper, penalty scenario and Genetic algorithm are used for alarm generation, and parameter optimization in Tennessee Eastman (TE) Process. The results of proposed method are compared with other methods.

Abstract: 

Strength measurement of rock requires testing that must be carried out on test specimens with particular sizes in order to fulfill testing standards or suggested methods. Often, the coring process breaks up the weaker core pieces, and they are too small to be used in either index tests or conventional strength tests such as point load index (Is) and Brazilian tensile strength (BTS). One of the index tests to indirectly determine the rock strength is the Block punch index (BPI) test, which requires flat disc specimens without special treatment. This study aimed to evaluate the applicability of the BPI test for predicting the uniaxial compressive strength (UCS), BTS and IS of the sandstones by empirical equations. Also, we have compared the performance of the BPI and IS for predicting the UCS and BTS. It was experimentally shown that BPI is a reliable method for predicting the UCS, BTS and Is of the sandstones under study. Moreover, the results indicate that BPI could be utilized with same importance as Is for predicting the UCS, while predicting the BTS by Is appears to be more reliable than BPI.

Abstract: 

Introduction: When it is desired to examine occurrence of two events simultaneously, it is common to use bivariate statistical models such as bivariate logistic regression. Due to the limitations of classical methods in real situations, other methods such as artificial neural networks (ANN) are concerned. The aim of this study was comparing the predictive accuracy of bivariate logistic regression and artificial neural network models in diagnosis of death occurrence and heart Block in myocardial infarction patients.Material and Methods: In this study, data was taken from a census in a cross-sectional study in which 263 patients with myocardial infarction cases who admitted to Hajar hospital heart care in 2013 to 2014. Gender, type of stroke, history of diabetes, previous history of hypertension, lipid disorders, history of heart disease, cardiac output fraction, systolic blood pressure, diastolic blood pressure, fasting and non-fasting blood sugar, cholesterol, triglycerides, low-density cholesterol, smoking, type of treatment, the troponin enzymes and insurant type were considered as explanatory variables and occurrence of death and heart Block were used as dependent variables. Bivariate logistic regression and neural network model was fitted. Both models were predicted and the accuracy of them were compared. Models were fitted by MATLAB2013a and Zelig in R3.2.2.Results: Predictive accuracy of bivariate logistic regression model was 77.7% for the training and 78.48% for the test data. In ANN model, LM and OSS algorithms had best performance with 83.69% and 83.15% predictive accuracy for training data and 84.81% and 83.54% for testing data, respectively.Conclusion: This research showed that the neural network method is more accurate than bivariate logistic regression to Joint predicting the occurrence of death and heart Block in patients with myocardial infarction.

Abstract: 

Ultrabasic rocks constitute a major part of ophiolites in south of Mashhad. Due to increase in the price of property and development of this city in southern highlands and also increasing construction in this area, the study of ultrabasic rocks and their Joint system, from the perspective of engineering properties assessment of the rock mass and its effect on the instability of trenches and slopes in this area, has been a matter of great importance. In this study, three different experimental methods have been used to calculate RQD and the results indicate a reasonable concordance. In addition to the determination RQD, the parameters such as the weighted Joint density (Wjd) and the Block volume (Vb) have been calculated that these methods offers higher specifications of Block size. The Block size is an important input data in many rock engineering calculations and can be measured using different experimental methods and modeling.

Abstract: 

Purpose: Together with the increase of orthodontic treatment needs the use of functional apparatus has been increased. Furthermore, different studies has been done on skeletal and dental changes after the use of these functional appliances, but few studies discussed soft tissue changes in the field. So, this study was carried out in order to evaluate soft tissue changes following the use of Twin Block functional appliance.Methods & Materials: This study was quazi experimental done on 13 patients (7 girls and 6 boys) with 7 years and 7 months to 12 years and 6 months of age. All patients were Cl II Div.I malocclusion, Iranian, Muslim, resident of Tehran and with no previous orthodontic treatment and tooth extraction. The period of treatment with the apparatus varied between 7 to 17 months. The studied angular variables were nasolabial, nasofacial, facial convexity,H angle, mentocervical, nasomental and mentolabial. The liner variables were lower and upper lip to E line, upper and lower lip thickness, prominence and length, upper and lower face height. All variables were compared before and after the use of Twin Block and recorded in a questionnaire and were analyzed by T - test. Results: The study showed that nasolabial, nasofacial, Lower lip to E line and Upper lip length decreased slightly with the use of apparatus, while facial convexity, H angle, upper lip prominence, upper lip thickness and upper lip to E line decreased significantly. As well as, mentocervical, upper lip thickness, lower lip prominence and soft tissue lower face height increased slightly and nasomental, mentolabial, soft tissue upper face height and lower lip length increased significantly.Conclusion: Following the treatment with Twin Block, the lower face soft tissue moved anteriorly leading to decrease in profile convexity. The upper and lower face soft tissue height increases and their ratio improved. The upper and lower lip thickness reduced, but upper lip length did not change and lower lip length increased after the treatment. The space between upper and lower lip edge to E - line moved to a normal range and labiomental fold became wider.
